A state of the art treatment that uses extreme cold, or cryotherapy, to painlessly and safely destroy unwanted, benign and superficial lesions on the surface of the skin, such as warts, skin tags, verrucae, milia, cherry angiomas, scars, and resistant hyperpigmentation, many of which are no longer able to be treated by doctors under the NHS.

It has four main benefits over other treatments;

Technical Specifications

CryoPen uses a technology referred to as cryotherapy – ‘cryo’ meaning ‘icy cold’. This involves the use of a high pressure jet of nitrous oxide – N2O, (the source of the cold energy), which is directed at the specific lesion being treated, to within millimetre accuracy.

How does it work?

CryoPen therapy is a medical procedure that involves the application of nitrous oxide gas ejected under high pressure on to the skin. The gas is very cold which helps to freeze and destroy any benign lesions.

During the application, the tip of the CryoPen reaches -89°C. The reason for this is to create a thermal shock at the region where the gas is pointed by causing a rapid drop in temperatures. The temperature drop has the effect of crystallising the fluid contents of the targeted cells, turning them into ice shards, resulting in the destruction of skin lesions as the membrane surrounding the treated cells is ruptured and destroyed.

During treatment, you may see a frost or ice-field on top of the lesion. This shouldn’t cause any worry as the frost usually goes away within a few seconds. A common technique called freeze-thaw-freeze can sometimes be used to freeze, then defrost a frozen lesion, and then freeze it again. Here, the frozen lesion is allowed to rest for 30 seconds and then a second treatment applied whereby the tissue will freeze faster second time around and destruction of the lesion is more guaranteed.

After treatment, it is advisable to leave the area uncovered as this speeds up the healing process. However, your practitioner may apply a simple dressing or plaster if there is a chance that the lesion may get rubbed or aggravated in any way. Dr Sally will advise you how to care for your skin post treatment.

Hyperpigmentation Treatments at SDS Rejuvenate MediSpa Wirral.

Hyperpigmentation is the result of excess melanin being produced in the skin, and it’s the reason a lot of clients visit our clinic for expert advice. Hyperpigmentation can appear anywhere on your body and appears as little patches of skin that are deeper in tone than the rest of your body, meaning that your skin tone might appear uneven. It’s super common in all ethnicities but is especially prevalent in those with darker skin tones.

When your skin is exposed to sunlight, the melanocytes in the deeper layers of your skin produce cells that contain a skin-darkening pigment called melanin. If certain areas of your skin overproduce melanin, you end up with darker patches of skin known as hyperpigmentation. And it’s not just the sun’s rays that cause these patchy dark spots – recent studies have also shown High Energy Visible light (HEV), which is omitted from your smartphone, is also a culprit. Bad news for those who stare at screens all day.

What are the different types of hyperpigmentation?

Sunspots

They appear randomly on all areas of the face as well as other parts of the body that are exposed to a lot of sunlight, such as the hands and back. They’re likely to appear alongside other signs of photodamage, such as textural changes to the skin like lines or wrinkles. This type of pigmentation is directly linked to sun exposure, rather than hormonal changes.

Melasma

It presents itself as light to dark brown patches on the forehead, cheeks, chin, upper lip and is very symmetrical in appearance on both sides of the face. The discoloration can lie on the epidermis (upper layer of skin), dermis (lower layer of skin), or a combination of both. It’s one of the most common skin concerns that women of colour face because they produce more melanocytes, the cells responsible for creating pigment.
Melasma can also be hormonally triggered and it linked to changes like pregnancy and menopause as well as the introduction of synthetic hormones via the contraceptive pill. Exposure to sunlight increases the intensity of melasma making it trickier to more difficult to get rid of.

Scarring and post-inflammatory hyperpigmentation

Also known as PIH, it often appears as the result of skin injury or an inflammatory skin condition. Think acne, chicken pox or dermatitis.

Rosacea Treatments at SDS Rejuvenate MediSpa Wirral.

Recent data reveals that rosacea affects 77% of people emotionally, 67% socially, 63% at work and 53% in their relationships or dating behaviour.

What are the symptoms and how do I know I have rosacea?

  • Frequent flushing of the face
  • Redness (erythema) which my look like sunburn
  • Small lumpy spots which may look like acne
  • Telangiectasia – tiny blood vessels under the skin
  • These symptoms are usually on the cheeks, forehead, nose and around the mouth and chin.

Who gets rosacea? – Around 1:10 people in the UK are thought to develop rosacea. Symptoms may first appear in your early 20’s, but the usual age that these symptoms appear is in your 30’s to 50’s. Women are more likely to suffer than men and it is more prevalent in fair skinned than dark skinned individuals.

What causes rosacea? – The exact cause of rosacea is unknown; a number of factors may be involved, e.g. sun damage, genetics, abnormal immune reactions in the skin which lead to inflammation.

What are the treatments available? – Unfortunately you cannot stop the development of rosacea but there are treatments that can help ease the symptoms.

General Measures:

Avoid – Strong sunlight to the face as it is thought to make symptoms worse. Using a good sun block 30+ with UVA and UVB protection.

Avoid – Extremes of temperature, alcohol, strenuous exercise, stressful situations, spicy foods, and hot drinks as these can all precipitate the facial flushing.

Laser – Using our powerful Fotona laser we can treat the dilated blood capillaries (thread veins) associated with rosacea, which aims to destroy the vessels by thermocoagulation.

Dermalux LED – This fantastic treatment is non-invasive, pain free and has no downtime. It harnesses the power of pure light to accelerate the skin’s natural rejuvenation and repair processes. The Menopause

Finally in 2022 that menopause has become a word we can freely use. Prior to this it would be behind a woman’s back one could be heard whispering “it’s because she’s going through the change” – the subject was taboo.

Today you only need to switch on morning TV, scroll through your device or pick up a magazine and someone somewhere is talking about the menopause. This has been highlighted by a number of well-known, well respected, high-profile celebrities such as Penny Lancaster, Davina McCall, Mariella Frostrup and Lisa Snowdon. They have openly discussed their personal issues and together have campaigned for reform in the menopause support at the House of Commons during the Menopause Mandate event in June this year.

The facts presented at the Menopause Mandate event include: – 44% of women said their ability to work has been affected – 61% said they had lost their motivation at work due to their symptoms – 52% said they had lost their confidence – one in 10 women who had been employed during menopause have left work due to symptoms – just 55% of women said they had talked to someone at their GP practice

These are frightening statistics, and this is why there is so much publicity… and rightly so.

This publicity and the programs that have been aired, I believe, have been incredibly helpful as I think many women have not realised what was causing their problems , they could not fathom out why they were feeling just “so awful”. This is particularly important when women are having these alien symptoms as early as in their 40s and at this point in their life they blame busy lives, young children, demanding jobs etc. and do not consider the symptoms may be due to the perimenopause

The perimenopause and menopause is a continuation of the ageing process of the ovaries and even in your 40’s the ovarian function is not quite as it was in your 20’s and 30’s. You may start to experience irregular periods, they may be heavier than usual, you may experience night sweats prior to your period and worsening PMS. Most of the time, most of the women “just get on with it “– but should you? Of course not!

Diagnosis of the perimenopause and menopause no longer necessarily requires having a blood test. The perimenopause can be diagnosed on the basis of vasomotor symptoms, e.g., hot flushes and sweats despite regular periods. Menopause is diagnosed when women have not had a period for at least 12 months and have not been using hormonal contraception.

As doctors it is important to follow the NICE guidelines (National Institute for health and care excellence) for the diagnosis and management of many medical conditions – menopause being one of them. The guidelines aim to improve the consistency of support and information provided to women during the perimenopause and menopause

Increased publicity about the menopause has also meant women are becoming more informed about available treatments that can help them make the transition through the perimenopause and menopause more comfortable.

The depletion of oestrogen, progesterone and testosterone during the perimenopausal period trigger changes to the hair. skin and body which leave a woman feeling less confident about herself. I cannot tell you how many times a woman comes into clinic and says, “I don’t know that person who looks back at me in the mirror anymore” , or “I have simply aged overnight “ They then look back at me across the desk and say “what has happened to me?“ “what can I do?”, “what can you do?”

Anyone who is suffering or who has suffered will recognise these typical signs when you look in the mirror -the increase in the lines and wrinkles, the crepey skin, the sagging and the jowling, the loss of those lovely plump cheeks and high cheekbones, even acne outbreaks and facial hair growth. All these are recognised signs of aging related to the depletion of our hormones, especially oestrogen.

It is not only the changes in the face that cause distress, but there are also many more issues to consider, some of which are less openly discussed.

Middle-age spread – this is a typical problem. These unwanted and seemingly unexplained changes in the body cause much distress and despite how hard a woman diets and exercises she just does not seem to win the battle of the bulge. The harsh reality is that our metabolism, digestive system and pretty much everything else is slowing down. Nothing prepares women for these visible changes in their external appearance

Hair loss and thinning- this does not just affect the scalp and the condition of the hair but also there can be depletion of eyebrows and eyelashes.

Intimate health issues – collagen loss happens everywhere and has a lot to answer for! Collagen loss in the intimate areas leads to dryness, tissue atrophy and discomfort during sex which then has an effect on loss of libido, recurrent urinary tract infections and urinary incontinence. These symptoms can have a very negative impact on a woman’s confidence and quality of life and can adversely affect relationships with their partner.

Disrupted sleep – women going through the menopause lose, on average, more than 5 1/2 weeks of sleep per year. A study conducted by sleep experts at Silentnight found 64% of women going through the menopause suffer from disrupted sleep. Sleeplessness can affect more than 10 different hormones and shifts in the levels of these hormones can cause changes in appetite, mental well-being and cardiac health.

Changes in mood – feeling tired, irritable, less tolerant, anxious, depressed, feeling of worthlessness, brain fog and I am sure there are many more adjectives I could add to this list. Not only does it personally distress the woman herself, it impacts on relationships at work, at home and in social circumstances.

I have only covered the most common signs and symptoms for anyone reading this who is going through the peri menopause or menopause you can probably add many more conditions and concerns. So enough about the symptoms, what can we do about it, how do you start to navigate the journey of the menopause, how do we make it a little easier to cope with– simply what can be done and who can help?

Even as a doctor who spent almost 30 years in general practice, I realise this is a very complex question and the answer is even more so. Complex though it is there are solutions available, but one has to realise a fix for one person is not necessarily a fix for another. We all have different medical histories, risks and medical problems and as such we must focus on individualising. HRT will always be something a woman immediately thinks about and this is still very important to consider when you are struggling with symptoms . Taking hormones in whatever form of delivery is not necessarily the whole answer but can be invaluable in many cases. However, it is now considered we should take a holistic approach to treating the perimenopause and menopause. There are many articles and experts who show the importance of mindfulness, the importance of health, nutrition, lifestyle etc. In the first instance we have to identify with each individual what is the main concern and then a package of treatments can be developed this will bespoke to each and every woman

As doctors both Ian and I have a long history of working in the NHS, myself as a GP and Ian as an endocrinologist but we now work full time in the speciality of aesthetics where we believe we can make a positive difference to women experiencing the effects of the perimenopause/menopause. Your skin

We can advise on skincare. After undergoing a skin analysis we can prescribe a comprehensive cosmeceutical skincare regime to really hydrate, brighten and firm your menopausal skin We may advise in-clinic treatments with the aim to stimulate collagen and elastin .e.g., using techniques such as micro needling ,Morpheus8, skin boosting injections and laser treatments ( the 4D facelift has long been a favourite in our clinic for lifting and tightening the skin on the face Menopause also causes areas of pigmentation to develop especially on the face and backs of hands. There are many treatments available to fade or even banish these unwanted “age spots” and these include cosmeceutical skincare, laser, Cryopen (liquid nitrogen) and skin peels

Volume loss in your face

Once we have addressed the skin laxity, we can further enhance your appearance using hyaluronic acid dermal fillers which subtly re-volumise areas in the face where you’ve lost fat pads and hence volume. This is subtle but addresses the tired look , the sunken cheeks, and the deeper and more marked lines in the lower face (nasolabial folds and marionette lines) The effect is immediate in that you can see the lifting and volumisation as you look in the mirror, but it is the boost to one’s confidence and self-esteem which is monumental and rewarding.

Unwanted hair

Laser and electrolysis can banish that facial hair you seem to have developed in these difficult times. A course of treatment is required but we can advise you fully on this after an initial consultation.

Your body

Unwanted changes in the body can be addressed with a combination of body contouring treatments. Using fat freezing techniques, we reduce the pockets of stubborn fat that just will not go despite diet and real effort of exercise. The crepiness of the skin can be improved using micro needling combined with radio frequency (Morpheus8). In some cases, radio frequency alone (Forma) or Laser technologies can be successful, particularly when treating the inner thigh and upper arm area.

Genitourinary symptoms of the menopause

These include vagina dryness, painful intercourse (dyspareunia), itching and the increased urge to pass urine. Topical treatments using pessaries or gel can help but the Fotona Laser can also offer a solution to your worries.

Now back to HRT – It is important to realise that Media reports about HRT have not always been accurate. It was stated HRT prevents osteoporotic fracture and colon cancer, but it also stated HRT increases the risk of having a cardiovascular event and can increase the risk of developing breast cancer. Association between HRT and cardio cardiovascular events has since been disputed and the risk of breast cancer depends on the type of HRT(oestrogen only is associated with little or no increased risk, HRT with oestrogen and progesterone can be associated with an increase.

It is therefore important you talk to your GP and air your concerns. Your GP will consider your medical history and if HRT is an option, they will discuss which preparation will suit you best. There are alternatives to HRT. The NICE guidelines encourage all women to be given information and advice on all available treatments. These include HRT, non-hormonal treatments and nonpharmaceutical treatments, e.g., cognitive behavioural therapy (CBT), they should be advised on complimentary therapies and the regulated preparations, (compounded bioidentical hormones). The long-term benefits and the risks of HRT should be made available, and any myths clarified.

Alternative measures to HRT include:

Tibolone (Livial is the brand name).

This is similar to take in combined HRT and is a prescription medication. It is particularly good at relieving symptoms such as hot sweats, low mood and reduction in sex drive. but can only be used in the post-menopausal woman (a woman who had the last period more than 12 months ago). It is a tablet once a day.

Antidepressants

Clonidine

Bio identical or natural hormones

Bioidentical hormones are plant based and may be considered as “natural or softer”. However, the NICE guidelines do not recommend these products as they are not regulated and there is no evidence, they are safer than standard HRT. The balance of hormone used in these preparations are based on hormone levels in your saliva but there is no evidence that these levels are related to your symptoms.

Body identical hormones or micronised progesterone

A common form of micronised progesterone prescribed is “Utrogestan”. This is a natural , derived from plants that are biochemically identical to the progesterone produced by your ovaries. This is sometimes called a “body identical” or “bio identical” hormone. Utrogestan taken in combination with oestrogen forms part of HRT.

Complimentary therapies

Common products purchased in health shops include St John’s wart, black cohosh, evening primrose oil and Angelica and ginseng. There is evidence that black cohosh and St John’s ward may help reduce hot flushes, but many complimentary therapies are not backed by scientific evidence. Warmest also be careful when considering the dose and what are the prescribed medications you’re taking. Natural does not always mean safe. The quantity purity and the ingredients cannot always be guaranteed.
